Sculptra/Lanluma

Sculptra is the commercial name for injectable PLL (Poli L-lactic acid) produced and distributed  by Galderma.

PLL is a biocompatible and biodegradable polimer.

It is a biostimulator, that is, a collagen stimulator; when injected in the cheek area, it has been clinically shown to reduce cheek wrinkles, improve firmness and quality of the skin.

Collagen is the main structural protein of our skin.

The amount of collagen in our skin decreases by 1 % per year starting from our mid twenties

Sculptra helps stimulate natural collagen production to help restore facial volume  and firmness over time.

The product differs from neuromodulators such as Botox and from hyaluronic acid fillers.

Botox reduces the intensity of repeated  facial muscle movements to help with dynamic lines and wrinkles.

Hyluronic Acid fillers are basically gels which add instant volume to sculpt and support the face.

Scupltra differs from Botox and traditional HA based fillers because it stimulates your body to produce your own collagen.

The vial contains dry powder which is composed of: PLL , Carboximethylcellulose and mannitol.

The product has to be reconstituted by adding sterile water for injections so that the PLL particles are suspended in a water solution.

So initial dilution and hydration of the PPL vial is necessary. We also add a bit of a numbing agent (such as lidocaine to make the injections more comfortable)

After being injected,  PLL induces a subclinical inflammatory response followed by encapsulation of the PLL particles and subsequent fibroplast (collagen producing cells in our skin) activation and proliferation with new collagen formation (neocollagenesis). PLL is gradually degraded and metabolized in Co2 and H2O

Ideal candidates for Sculptra injections are:

Patients wanting natural results  over time.

Patients who loose facial volume and facial fat with age, who look a bit hollow.

Patients who need pan-facial improvement, that is additional volume restoration and wrinkle correction everywhere in the face,

Sculptra acts differently than traditional injectable fillers because new volume is generated in a gradual, progressive manner by your own  skin.

The results are definitely more natural and last longer over 2  years.

Where is Sculptra injected?

It is injected in the deep dermis /subcutaneous layer with a crosshatching technique with either a needle or a cannula (which is a blunt needle)  on the mid cheek , lateral cheeks and preauricular area

It can also be injected supra-periosteal (deep) in the temple, zygoma and and mandibular area, and at the base of the nose.

Typically patients require 1 to 2 vials per session and 3 sessions for full treatment.

Treatments should be at least 6 weeks apart.

It is estimated that you require 1 vial of Scultpra for every decade of life.

Patients in their 30s would need 3 vials , patients in their 40s require approximately 4 vials, etc.

In a single session we inject no more than 2 vials.

Massage is required after injection  in order to prevent formation of Sculptra nodules, majority of which dissolve on their own without any intervention.

Patients should also use the “rule of fives”, that is, massage the treated areas for 5 minutes, 5 times daily for 5 days, in order to allow even distribution of Scupltra particles.

Scupltra should not be used in patients with a history or keloid or hypertrophic scarring.

Swelling, redness, irritation, bruising are the most common side effects.

Results take time, 3 to 6 months; typically patients start noticing a difference after their second session.

With Scupltra, you are never going to look unnatural, puffy, full or overfilled.
